Vulnerabilities are discovered in software constantly, and attackers move quickly to exploit them. Patch and vulnerability management is the continuous process of finding, prioritizing and fixing weaknesses. Change management is the discipline that ensures fixes, and every other modification, are made safely. The two are tightly linked: a patch is a change, and an unmanaged change can create new vulnerabilities or outages.
